Collect: Error when trying to upload a csv grid to a survey

An error appears when selecting the "type" of a file to be attached to the survey. The same error also appears when selecting the csv file (grid) and it is not possible to proceed with the file upload.

ERROR: "org.openforis.collect.designer.form.SurveyFileFormObject_$$_jvst5f6_3 cannot be cast to org.zkoss.bind.SimpleForm"
