Can not be created a new survey

Dear OFT, I am survey adminstrator. I want to create a survey and design a form as per my need. But I am unble to create a survey. I remember I could do the same in previous months.I found tha t Now, neither I can create a survey nor assign any user as survey manager.Now, My main qustion is that how can I create a new survey? Thanks in advance.
