Unexpected error: error: syntax error at or near "end"

I am trying to publish a survey for the first time with Arena online. I am seeing this error message under "12. Survey RDB Creation" and it refers to "Item" 4 as where the error is located. What does this error message mean? What does "Item" refer to?
