error - multiple nodes found for a path

So I have a code attribute set to multiple (min count 1 and max 4), always relevant. Everything looks good in the survey designer, but when using Collect Mobile it gives me an error - Multiple nodes found for path: / plot-data/hist_code and it won't export the data. It also had an issue when I included Specify field in the code list. At this point I just removed it, however, I need to bring back the specify field. The app also does not allow me to open the record containing that error once I closed it. Any suggestions?
